Estos par\u00e1metros pueden permanecer elevados incluso cuando se alcanza el objetivo de C-LDL. Por ejemplo, en un an\u00e1lisis en el que se utilizaron datos de la Encuesta Nacional de Examen de Salud y Nutrici\u00f3n y el estudio Amplia Base de Datos de L\u00edpidos, entre uno de cada siete y uno de cada 12 pacientes de alto riesgo con el objetivo de C-LDL (&lt;70 mg\/dl) ten\u00edan niveles elevados de colesterol no HDL y apoB, respectivamente 5. Por ese motivo, se propusieron los dos componentes como potenciales marcadores para evaluar el riesgo cardiovascular residual. Sin embargo, en los lineamientos sobre la dislipidemia de la Sociedad Europea de Cardiolog\u00eda\/Sociedad Europea de Aterosclerosis de 2019, se concluy\u00f3 que la apoB era una medida m\u00e1s precisa del riesgo cardiovascular y una mejor orientaci\u00f3n para la idoneidad de la reducci\u00f3n de l\u00edpidos respecto del C-LDL o el C-HDL 4 .<\/p>\n<p style=\"text-align: left;\">De hecho, existe una fundamento cient\u00edfico que respalda esta propuesta. La apoB es el principal componente estructural de las lipoprote\u00ednas aterog\u00e9nicas, incluidas las LDL, las lipoprote\u00ednas ricas en triglic\u00e9ridos y sus residuos. Dado que solo hay una mol\u00e9cula de apoB en cada part\u00edcula de lipoprote\u00edna que contiene apoB, la medici\u00f3n de apoB sirve para cuantificar la cantidad total de part\u00edculas aterog\u00e9nicas que contienen apoB. Por lo tanto, la apoB integra los marcadores de l\u00edpidos convencionales (colesterol total, triglic\u00e9ridos, C-LDL y colesterol no HDL) en un solo \u00edndice. Los estudios epidemiol\u00f3gicos y los estudios de aleatorizaci\u00f3n mendeliana demuestran que la apoB comparada con el C-LDL o el C-HDL es un predictor superior del riesgo cardiovascular asociado con las lipoprote\u00ednas aterog\u00e9nicas que contienen apoB 6-9. Estos resultados implican que la apoB puede ser el principal causal de la aterosclerosis y que la reducci\u00f3n de la concentraci\u00f3n de todas las lipoprote\u00ednas que contienen apoB deber\u00eda ser el enfoque principal de las estrategias terap\u00e9uticas para reducir el riesgo relacionado con las lipoprote\u00ednas.<\/p>\n<p style=\"text-align: left;\">M\u00e1s all\u00e1 de su utilidad como marcador de riesgo cardiovascular residual, la apoB tambi\u00e9n puede orientar la intervenci\u00f3n terap\u00e9utica. Un an\u00e1lisis reciente de ODYSSEY OUTCOMES destac\u00f3 ambos atributos 10. En este ensayo, se asignaron al azar a pacientes con s\u00edndrome coronario agudo reciente y niveles elevados de lipoprote\u00ednas aterog\u00e9nicas (C-LDL \u226570 mg\/dl [1,81 mmol\/l], colesterol no HDL \u2265100 mg\/dl [2,59 mmol\/l] o apoB \u226580 mg\/dl) a pesar de estar recibiendo estatinas de alta intensidad y tratamiento con alirocumab o placebo. El riesgo de un episodio cardiovascular grave (MACE) no solo aument\u00f3 progresivamente a medida que crec\u00edan los niveles de apoB iniciales (en un 11 % por cada incremento de 10 mg\/dl en la apoB inicial), sino que los pacientes con los niveles m\u00e1s bajos de apoB durante el tratamiento alcanzaron mayores reducciones relativas y absolutas en los MACE a los 4 meses. Desde la perspectiva de la cl\u00ednica, la pregunta es cu\u00e1l es la mejor manera de integrar este conocimiento en la pr\u00e1ctica habitual. La medici\u00f3n de apoB ahora puede realizarse f\u00e1cilmente, es econ\u00f3mica y est\u00e1 comprobada, no requiere un an\u00e1lisis de sangre en ayunas y puede incorporarse habitualmente en los lipidogramas cl\u00ednicos 11.
